A representative from Mom’s Home Care can meet with you and/or your loved one in the comfort of your own home to discuss care needs and services. This service is offered free of charge.
At this consultation, we will set up a care plan based on your needs. This will include:
- General Client Information
- Days and hours of service required
- General Instructions
- Health history
- Medications
- Details of daily assistance (personal care, housekeeping, transportation, meal preparation etc.)
- Cognitive limitations
- Other preferences
In addition, we will discuss our policies, procedures, other services, and rates. There is absolutely no commitment required to use our service.
